Our process follows FMCSA and NRCME requirements.
We will help you understand what to bring to your appointment.
The exam typically takes 30 to 45 minutes and covers a structured set of health assessments designed to ensure you can safely operate a commercial vehicle. Your provider will review your medical history and conduct the following:
If you are found medically qualified to drive, your provider will complete and sign the Medical Examiner's Certificate. If a condition is identified that requires follow-up before certification can be issued, your provider will let you know what documentation or treatment is needed.
Coming prepared helps your visit go smoothly and reduces the chance of needing to return. Bring the following to your appointment:
If you have been treated for insulin-dependent diabetes mellitus and are seeking an exemption to operate a CMV, your treating provider will need to complete the FMCSA MCSA-5870 form. You can download the MCSA-5870 form directly from the FMCSA or through our DOT physical page.
DOT physicals at Family First Chiropractic are $100. Because DOT physicals are classified as occupational exams rather than medical treatment, they are not covered by health insurance. Payment is due at the time of service.
Please note: the exam fee is charged regardless of the outcome. If you are found medically unqualified during the examination, the fee is non-refundable. If a disqualifying condition is identified, your provider can discuss what steps — such as treating a condition or obtaining a waiver through the FMCSA — may allow you to become qualified in the future.
Failing a DOT physical doesn't always mean a permanent disqualification. Many conditions can be managed or documented in a way that still allows certification. Common reasons drivers do not pass include:
If you have a medical condition you are concerned about, bring documentation from your treating provider. Our medical examiners review all submitted records as part of the evaluation and will tell you clearly what, if anything, needs to be addressed before a certificate can be issued.
Most commercial drivers are required to renew their DOT medical certification every two years. However, if you have a medical condition that requires monitoring — such as hypertension or diabetes — your medical examiner may issue a shorter certification period (for example, one year or six months) to allow for follow-up before the next renewal. The certification period is determined by your medical examiner based on your individual health profile.
In addition to federally required DOT physicals, Family First Chiropractic offers non-DOT employment physicals for employers and workers in industries that require occupational health screenings but are not subject to FMCSA regulations. These exams are commonly required for:
Non-DOT physicals are tailored to your employer's requirements. Call us to discuss what your specific exam needs to include and we'll make sure you're properly cleared for the job.
